    Isogeometric methods are extended for the parametric design process of complex lightweight structures. Three novel methods for the coupling of different structural elements are proposed: rotational coupling, implicit geometry description, and frictionless sliding contact. Moreover, the necessary steps for the integration of the numerical analysis, including pre- and post-processing, in CAD are investigated. It is possible to base several different analyses on each other in order to parametrically represent a construction process with multiple steps.Die isogeometrischen Methoden werden zur Anwendung im parametrischen Entwurfsprozess von komplexen Leichtbaustrukturen erweitert.     Bacterial fouling on surfaces significantly increases the resistance of bacteria toward antibiotics, which leads to medical complications and a corresponding financial burden. Here, we report on a general and robust technique for facile modification of various surfaces with different antibacterial agents. Our approach in this study was inspired by the strong adhesion of mussel adhesion proteins (MAPs) to many types of surfaces, including metals, polymers, and inorganic materials. Thus, glass and polymeric slides were dip-coated with dopamine, as a MAP mimic, and the resulting surfaces were characterized. The reactivity of dopamine-coated surfaces toward nucleophilic addition was then confirmed by reacting them with fluorescent probes containing either a free amino or a free thiol group. Laser scanning confocal microscopy (LSCM), X-ray photoelectron spectroscopy (XPS), confocal Raman microscopy, matrix-assisted laser desorption/ionization-time of flight (MALDI-TOF) mass spectroscopy, and cyclic voltammetry studies collectively suggested that the probes had covalently attached to the surfaces. Fabrication of dopamine-coated surfaces with an antibacterial quaternary amine or an ultrashort lipopeptide analog generated surfaces that effectively kill Escherichia coli and Staphylococcus aureuscells on contact. Moreover, minimal leaching of the fabricated agent was detected after prolonged incubation. This technique could be further developed to a "paint-like" or self-assembling monolayer-like procedure for the preparation of antibacterial surfaces on various materials

    Learning signals of adverse drug-drug interactions from the unstructured text of electronic health records.

    Drug-drug interactions (DDI) account for 30% of all adverse drug reactions, which are the fourth leading cause of death in the US. Current methods for post marketing surveillance primarily use spontaneous reporting systems for learning DDI signals and validate their signals using the structured portions of Electronic Health Records (EHRs). We demonstrate a fast, annotation-based approach, which uses standard odds ratios for identifying signals of DDIs from the textual portion of EHRs directly and which, to our knowledge, is the first effort of its kind. We developed a gold standard of 1,120 DDIs spanning 14 adverse events and 1,164 drugs. Our evaluations on this gold standard using millions of clinical notes from the Stanford Hospital confirm that identifying DDI signals from clinical text is feasible (AUROC=81.5%). We conclude that the text in EHRs contain valuable information for learning DDI signals and has enormous utility in drug surveillance and clinical decision support

    Physiological and genetic description of dissimilatory perchlorate reduction by the novel marine bacterium Arcobacter sp. strain CAB.

    A novel dissimilatory perchlorate-reducing bacterium (DPRB), Arcobacter sp. strain CAB, was isolated from a marina in Berkeley, CA. Phylogenetically, this halophile was most closely related to Arcobacter defluvii strain SW30-2 and Arcobacter ellisii. With acetate as the electron donor, strain CAB completely reduced perchlorate (ClO4(-)) or chlorate (ClO3(-)) [collectively designated (per)chlorate] to innocuous chloride (Cl(-)), likely using the perchlorate reductase (Pcr) and chlorite dismutase (Cld) enzymes. When grown with perchlorate, optimum growth was observed at 25 to 30°C, pH 7, and 3% NaCl. Transmission electron microscopy (TEM) and scanning electron microscopy (SEM) preparations were dominated by free-swimming straight rods with 1 to 2 polar flagella per cell. Strain CAB utilized a variety of organic acids, fructose, and hydrogen as electron donors coupled to (per)chlorate reduction. Further, under anoxic growth conditions strain CAB utilized the biogenic oxygen produced as a result of chlorite dismutation to oxidize catechol via the meta-cleavage pathway of aerobic catechol degradation and the catechol 2,3-dioxygenase enzyme. In addition to (per)chlorate, oxygen and nitrate were alternatively used as electron acceptors. The 3.48-Mb draft genome encoded a distinct perchlorate reduction island (PRI) containing several transposases. The genome lacks the pcrC gene, which was previously thought to be essential for (per)chlorate reduction, and appears to use an unrelated Arcobacter c-type cytochrome to perform the same function. IMPORTANCE The study of dissimilatory perchlorate-reducing bacteria (DPRB) has largely focused on freshwater, mesophilic, neutral-pH environments. This study identifies a novel marine DPRB in the genus Arcobacter that represents the first description of a DPRB associated with the Campylobacteraceae. Strain CAB is currently the only epsilonproteobacterial DPRB in pure culture. The genome of strain CAB lacks the pcrC gene found in all other DPRB tested, demonstrating a new variation on the (per)chlorate reduction pathway. The ability of strain CAB to oxidize catechol via the oxygenase-dependent meta-cleavage pathway in the absence of external oxygen by using the biogenic oxygen produced from the dismutation of chlorite provides a valuable model for understanding the anaerobic degradation of a broad diversity of xenobiotics which are recalcitrant to anaerobic metabolism but labile to oxygenase-dependent mechanisms

    Can I Be Myself Around Natives? Feelings of Inauthenticity Mediate the Relationship Between Perceived Discrimination and Tendencies to Avoid Friendships With Natives Among Refugees in Germany

    The social integration of the ever-growing number of refugees in receiving societies is of major importance. Perceived discrimination has been found to predict fewer friendships with natives over time. But what short-term mechanisms explain this effect? In a sample of 115 refugees living in Germany we (i) replicated the long-term discrimination-social-integration relationship, (ii) found short-term associations between discrimination and affective, motivational, and behavioral tendencies not to befriend natives, and (iii) showed authenticity to mediate this short-term relationship: with increasing discrimination, refugees felt less like they could show their authentic selves around natives, which in turn impaired tendencies to befriend natives. Discrimination may impede the formation of interethnic friendships by instilling feelings of inauthenticity. Implications for prevention measures are discussed

    Profiling risk factors for chronic uveitis in juvenile idiopathic arthritis: a new model for EHR-based research.

    BackgroundJuvenile idiopathic arthritis is the most common rheumatic disease in children. Chronic uveitis is a common and serious comorbid condition of juvenile idiopathic arthritis, with insidious presentation and potential to cause blindness. Knowledge of clinical associations will improve risk stratification. Based on clinical observation, we hypothesized that allergic conditions are associated with chronic uveitis in juvenile idiopathic arthritis patients.MethodsThis study is a retrospective cohort study using Stanford's clinical data warehouse containing data from Lucile Packard Children's Hospital from 2000-2011 to analyze patient characteristics associated with chronic uveitis in a large juvenile idiopathic arthritis cohort. Clinical notes in patients under 16 years of age were processed via a validated text analytics pipeline. Bivariate-associated variables were used in a multivariate logistic regression adjusted for age, gender, and race. Previously reported associations were evaluated to validate our methods. The main outcome measure was presence of terms indicating allergy or allergy medications use overrepresented in juvenile idiopathic arthritis patients with chronic uveitis. Residual text features were then used in unsupervised hierarchical clustering to compare clinical text similarity between patients with and without uveitis.ResultsPreviously reported associations with uveitis in juvenile idiopathic arthritis patients (earlier age at arthritis diagnosis, oligoarticular-onset disease, antinuclear antibody status, history of psoriasis) were reproduced in our study. Use of allergy medications and terms describing allergic conditions were independently associated with chronic uveitis. The association with allergy drugs when adjusted for known associations remained significant (OR 2.54, 95% CI 1.22-5.4).ConclusionsThis study shows the potential of using a validated text analytics pipeline on clinical data warehouses to examine practice-based evidence for evaluating hypotheses formed during patient care. Our study reproduces four known associations with uveitis development in juvenile idiopathic arthritis patients, and reports a new association between allergic conditions and chronic uveitis in juvenile idiopathic arthritis patients

    Network analysis of unstructured EHR data for clinical research.

    In biomedical research, network analysis provides a conceptual framework for interpreting data from high-throughput experiments. For example, protein-protein interaction networks have been successfully used to identify candidate disease genes. Recently, advances in clinical text processing and the increasing availability of clinical data have enabled analogous analyses on data from electronic medical records. We constructed networks of diseases, drugs, medical devices and procedures using concepts recognized in clinical notes from the Stanford clinical data warehouse. We demonstrate the use of the resulting networks for clinical research informatics in two ways-cohort construction and outcomes analysis-by examining the safety of cilostazol in peripheral artery disease patients as a use case. We show that the network-based approaches can be used for constructing patient cohorts as well as for analyzing differences in outcomes by comparing with standard methods, and discuss the advantages offered by network-based approaches
